Procedure

Poland is not a party to the Strasbourg Agreement on the Transfer of Corpses (1973), so the national rules of Poland apply, together with the requirements of the consulate of the Republic of Moldova. As a rule, the mortuary passport is then issued by an embassy or consulate of the Republic of Moldova.

We confirm the Polish procedure for taking a body abroad and the responsible office at the start of each case, and tell you which documents need your signature.

Required documents

Usually needed: the Polish death certificate, the passport of the deceased, the documents of the applicant and the certificates of the funeral service. We confirm the full list for your case.

The Polish sanitary permit

To take out of Poland the body of a person who died there, the form published on gov.pl requires a permit from the county sanitary inspector (państwowy powiatowy inspektor sanitarny). The permit is issued once the documents of the authority of the state where the burial will take place, that is Moldova, and of the transit states, showing that there are no obstacles, have been submitted.

These documents must be translated into Polish by a sworn translator (tłumacz przysięgły). The application may be filed by the spouse, relatives in the direct line, collateral relatives up to the fourth degree, in-laws in the direct line up to the first degree, or a person who voluntarily undertakes the obligation. The death record or another official document confirming the death is attached, and, if the cause of death is not stated, a document excluding an infectious disease.

Poland issues a short extract of the death record, the first one free when the death is registered; a multilingual extract also exists, on paper only. Poland is a party to the Apostille Convention (in force from 14.08.2005), and the Republic of Moldova since 16.03.2007.

We coordinate the Moldovan mortuary passport and the documents from Moldova, so that they reach the sanitary inspector in the required form.

With the family we choose the means of transport, plane or car, and the place of handover in Moldova.
